Requesting care should never feel like a maze. Complete the form with the older adult’s name, a reliable email, a preferred day and time, and a short note about what has changed — pain, memory, breathlessness, a recent fall, a hospital discharge, or a wish for a full geriatric review.
A coordinator reads every message personally. If the first-choice clinician is fully booked, we will offer the next suitable specialist rather than leaving the request unanswered. You will receive a confirmation by email. No payment is taken through this page; it is a request, not a checkout.

Medicines, recent illnesses and what a good week looks like at home all help us prepare.
2. Receive Confirmation
We write back with the clinician, the visit type and any notes to bring, such as previous reports.
3. Attend With Support
A trusted family member is welcome. Extra time is built in so questions are never hurried.
